About SOMOS Innovation



SOMOS Innovation is a full implementation of the holistic care model that was crucial to the success of SOMOS as a Performing Provider System in the DSRIP program. SOMOS Innovation is the next step on the path to culturally competent Value-Based healthcare. 


By utilizing the SOMOS Community Care model, SOMOS physicians reduced preventable hospital admissions by more than 20 percent, and avoidable readmissions by 25 percent. Given this remarkable performance, New York State’s Value-Based Payment (VBP) program designated SOMOS IPA as a VBP Innovator; SOMOS IPA is the only physician-led organization to earn that distinction.


SOMOS Innovation will enable healthcare providers to continue this trajectory by placing them in an optimal position to deliver affordable and cost-saving, high-quality, comprehensive care to the neediest patients, in accord with New York State’s Innovator Roadmap.

SOMOS Community Care is a not-for-profit organization comprised of a network of more than 2,500 independent physicians that care for nearly one million Medicaid patients, predominantly of Hispanic and Asian ethnicity in 4 boroughs of New York City and Nassau County. 


SOMOS IPA provides the management, administration and other services for the network to operate efficiently. Among other services, SOMOS IPA administers, provides and facilitates transactions between medical providers, patients, and third parties concerning health care services and benefits. Somos IPA also provides insurance claim administration services.

Under the leadership of Dr. Ramon Tallaj, Chairman of the Board and our CEO John Burke and his team, SOMOS physicians provide primary care and telemedicine visits to patients with significant access and language barriers.
