Shariff De Los Santos

Shariff De Los Santos is Vice President of Practice Transformation at SOMOS Community Care. His strategies and performance measurement implementation methods successfully transform Fee for Service practices into Value-Based Performance practices.
The performance measurements that Shariff and his team implement in SOMOS Community Care’s network practices enable them to close gaps in care, so they can thrive in a Value-Based Payment health care system.
A native of the Dominican Republic, Shariff graduated from the Universidad Iberoamericana (UNIBE) with a degree as a medical doctor. Shariff spent three years as a general practitioner in the Dominican Republic before moving to the United States.
At Somos Community Care, Shariff worked as the Cardiovascular Project Manager for almost two years before being promoted to Director of the Project Management Office. Besides his medical degree, Shariff holds certifications in Essentials of Project Management and Mastering in Healthcare Project Management from Villanova University.
In 2017 Shariff was promoted to Vice President Of Practice Transformation and was appointed as responsible for the implementation of Evidence-Based Best Practices at the Value-Based Payment Pilot group. Shariff and the teams he oversees have vastly improved the quality of the the practices they serve, resulting in a decrease in the gaps in care of the population they work with.
