Eric Van Meter

DIRECTOR OF PCMH

Eric Van Meter is the Director of SOMOS’ Community Care’s Patient-Centered Medical Home (PCMH) program, a team-based model of care led by a personal physician. Eric is responsible for the organization of PCMH and communications with our vendors. The PCMH vendors, providers and Eric build and maintain strong relationships to ensure the PCMH model is being implemented to the top tier of the providers practice. The medical home is best described as a model or philosophy of primary care that is patient-centered, comprehensive, team-based, coordinated, accessible, and focused on quality and safety.

PCMH has become a widely accepted model for how primary care should be organized and delivered throughout the health care system. A medical home is a place where patients are treated with respect, dignity, and compassion; it enables strong and trusting relationships with providers and staff. Above all, the medical home is not a final destination instead. It is a model for achieving primary care excellence so that care is received in the right place, at the right time, and in the manner that best suits a patient’s needs.

Eric works closely with physicians to improve the quality of services they provide to their patients. Eric began his career at SOMOS Community Care as a PCMH Engagement Specialist and was promoted to Director of PCMH early 2018. He has five years corporate sales experience and a background of nutrition and exercise. He has helped many clients achieve their health goals as a certified personal trainer for the past 4 years.
