What is a Social Care Network?

We are here to help you live a better life. We identify your needs and connect you with the best help possible.

From Housing to transportation, to healthy groceries and medical services. We are creating a movement that takes care of ALL NEW YORKERS, EQUALLY!

Medicaid is a government health insurance program that provides coverage for low-income individuals and families. As part of its comprehensive benefits, Medicaid includes social care services to address the social factors that can impact health outcomes.

In New York, all Medicaid members are eligible for Social Care HRSN (Health-Related Social Needs) services. This means that if you are enrolled in the Medicaid program, you can receive help with social needs that impact your health.

Understanding HRSN Services

HRSN stands for Health-Related Social Needs:
These are situations in your daily life that can affect your health, but aren’t directly medical, like having a safe place to live, access to healthy food, being able to pay for the things that you need, and being able to get around.

How does a SOMOS Social Care Network work?

OUR SCN SUPPORTS YOU IN DIFFERENT WAYS

By working with Community Based Organizations (CBOs) and other health, behavioral health and social care providers, as your SCN we will be responsible for connecting you with the care you need.

Screening

We connect you with a team member who will ask you some questions and tell you services you may need and qualify for.

Health Related Social Needs Services (HRSN)

After the screening, we will tell you what you qualify for and work to help you receive those services.

Social Care Navigation

Now that you know how we can help you, it’s time to connect you with the help you need.

Finding or keeping housing and assistance with improvements to the place where you live in now.

Transportation

Help with getting to appointments or other places to get the help you need.

Nutrition

Help with getting appropriate and healthy food

Case management

Personalized help with applying and getting the services that you are eligible for.

Eligibility

All medicaid members are eligible for screening. Medicaid members that are enrolled in a plan with a Managed Care Organization (MCO) may be eligible for additional services.

Members enrolled in direct medicaid may have limited services.

HRSN screening and services: Member Journey

Did you know there’s someone on your Medicaid team who can help with more than just medical bills? They’re called Social Care Navigators, and they’re here to connect you with resources that can improve your overall health and well-being. Here’s how:

We listen and understand

Your navigator will talk with you to see if you’re facing any challenges outside of doctor’s visits. This could be anything from finding affordable housing to getting healthy food on the table or having reliable transportation to appointments.

We connect you
to resources

Once we understand your needs, we’ll work to find the right programs and services in your community that can help. This might include food banks, job training programs, or even help with childcare.

We work with
your doctor

From coordinating social services with your medical treatment plan, to connecting with your doctor, our navigator will make sure you get the best possible care.

We fight for you

Navigator’s goal is to make sure you stay healthy and independent, therefore they advocate for you if you’re facing any difficulties accessing the services you need.
