As SOMOS Community Care providers, you play a crucial role in connecting patients to the social determinants of health (SDOH) services they need through the New York 1115 Waiver’s Social Care Network (SCN) ecosystem.
What is the SCN ecosystem?
The SCN ecosystem is a network of community-based organizations (CBOs) strategically chosen across regions in New York. These CBOs collaborate with SOMOS and its independent network providers to identify patients with unmet SDOH needs. These needs can encompass various areas, including housing instability, food insecurity, transportation issues, and lack of legal assistance.
How does it work?
Once you identify a patient with potential SDOH needs, you can:
- Complete the AHC screening tool: This tool helps assess various SDOH factors and identify areas where your patient or community member might benefit from additional support.
- Refer your patient/member: Through an integrated technological platform, SOMOS will be able to connect the patient or member to appropriate community resources and services dependent on their specific needs and track their progress.
