FINDHELP

This Website contains a link to FINDHELP.  You can receive screening for health-related social needs on the FINDHELP site.  On that site you will direct how your personal and other information may be used.  All information collected will remain within FINDHELP and with your permission will be shared with the Statewide Health Information Network for New York (SHIN-NY).  All health care providers that lawfully access the information you have provided on FINDHELP (with your permission) must comply with among others, federal and state privacy laws and regulations.  If health care providers affiliated with Somos Community Care access your data on FINDHELP (with your permission), such providers must comply with, among others, the privacy policies established by the Bronx RHIO and SHIN-NY.  On FINDHELP you may review or correct any of your personal information.  In addition, you may modify any consent to disclosure that you have given.

Unfortunately, the transmission of information via the internet is not completely secure. While we do not collect personal information through this Website, we cannot guarantee the security of the information you provide to us on our Website. Any transmission of information is at your own risk. We are not responsible for circumvention of any privacy settings or security measures contained on the Website.
