The health disparities in New York City’s underserved communities are stark. Medicaid patients and residents of New York City’s most underserved neighborhoods and communities face higher rates of chronic conditions, hospitalizations, and premature death compared to their affluent counterparts. These deeply rooted injustices are driven by systemic inequities in access to affordable healthcare, housing, healthy food options, and economic opportunities.

At SOMOS Community Care, we seek to collaborate with our dedicated network of independent providers and community-based organizations (CBOs), to dismantle the barriers to health equity and empower underserved New Yorkers to live their healthiest lives.
