There are systemic differences in the opportunities our community has to achieve optimal health, leading to unfair and avoidable differences in their health outcomes and overall well-being.

SOMOS tackles deep structural health inequities in our communities by building capability and designing innovative transformations for a robust network of independent community-based clinical and social care providers in New York.

SOMOS and its comprehensive network of providers are actively working towards achieving optimal, whole-person health and well-being within the broader social, political, and economic environment of our communities.
