Jimmy Ha
VICE PRESIDENT OF PRACTICE TRANSFORMATION
Jimmy Ha is Vice President of Practice Transformation and the Director of Provider Operations at SOMOS Community Care. Jimmy has more than 10 years of experience in health care administration and managed long-term care. He has spearheaded the implementation of various CMR software to track the daily operational activities of SOMOS. Jimmy has also adapted Salesforce to close Care Gap Measures for providers and utilizes it to assist with practice transformation through care coordination and value-base care
At SOMOS Community Care Jimmy Ha works with Independent Physician Associations (IPAs) serving providers in Asian communities. Jimmy leads four Practice Transformation teams that help execute SOMOS’ vision of serving our communities and providing quality care. The strategies and operational tactics his teams implement in the practices of SOMOS providers equip them to face the changes in health care delivery, as the health care industry transitions to Value-Base Payments and Coordination.
Prior to joining SOMOS Community Care, Jimmy was a Territory Manager at CenterLight healthcare. At CenterLight he helped build the Asian Provider network through fostered relationships with IPAs and their providers. Jimmy also facilitated the Managed Long-Term Care (MLTC) expansion into Westchester County, Nassau, and Suffolk Counties.
Before CenterLight Healthcare, Jimmy worked at Amerigroup as a Provider Relations Representative managing the Asian communities within the five boroughs of New York City.
