Jacqueline Delmont
MD, MBA, FCP, INTERNIST
Dr. Jacqueline Delmont, M.B.A., a Venezuelan native, is the Founder, President and Chief Executive Officer of Delmont Medical Care and Delmont Healthcare Consultants.
In 2000, Dr. Delmont founded Delmont Medical Care, a multi-site, multi- specialty, patient-centered medical home level 3 practice with 20 primary care providers, delivering culturally sensitive, ambulatory, efficient and high-quality medical care to 25,000 underserved populations across Long Island and Queens, insured predominantly by Medicaid (60%).
Between 2012 and 2015 Dr. Delmont was the Chief Medical Officer at Beacon Health Partners, a Long Island based Independent Practice Association and Accountable Care Organization with over 1000 physicians participating in the Medicare Shared Savings program and commercial payer contracts. As Chief Medical Officer, Dr Delmont was responsible for care coordination, disease management, quality assurance and compliance requirements. In addition, she established and implemented clinical guidelines and patient-centered medical home transformation.
In July 2019 Dr. Delmont joined SOMOS Innovation IPA as the Chief Medical Officer, overseeing 2,500 independent physicians, representing 800 practices and almost a million Medicaid lives. Together with the leadership team, Dr. Delmont is implementing the processes necessary to succeed at full-risk and partial- risk contracts across the spectrum of IPAs and lines of business.
Since March 2020 and throughout the COVID-19 pandemic, Dr. Delmont led SOMOS’s efforts of providing COVID-19 testing and immunizations to underserved communities, setting up and overseeing more than 200 testing and vaccination sites in collaboration with houses of worship, NY City and NY State leadership, Latin-American consulates, first responders, hospitals, and private organizations. Also, she has spearheaded efforts to improve physician accessibility, telemedicine visits, office re-openings, and physician financial sustainability.
Dr. Delmont became Board Certified in Internal Medicine both in her native Venezuela in 1995 and the United States in 1999. In 2012, she was chosen to participate as Health IT Fellow in the ONC Health IT Vanguards program. In 2013, Dr. Delmont completed her master’s degree in Business Administration at Adelphi University.
