To sustain the network providers’ ability to engage in effective, targeted preventive and whole-person care SOMOS:
Supports a network of clinical and social providers to collaboratively provide 360 care to community members.
Secure bidirectional exchanges and critical connectivity, centralized Information Technology (IT) system, and enhanced the Electronic Health Record (EHR) systems, and provide robust data collection and analytics thereby allowing for the application of quality metrics and performance measures.
Provides critical practice transformation and capacity-building activities for its network providers including expanding Patient-Centered Medical Homes (PCMH), assisting in closing care gaps, care management, care coordination, and deploying Community Health Workers (CHWs).
Reaches, meaningfully engages, and sustains the active participation of patients in their health, by engaging in advertising, marketing, communications, education, events, digital marketing, patient-use tools, health fairs and events, research and data, and strategic partnerships.
