That’s why we envision a dynamic collaboration hub where diverse stakeholders come together to break down silos and address the root causes of these disparities
How the Collaboration Hub Works
The hub serves as a platform for data sharing and analysis. Together, we’ll gain a deeper and shared understanding of the social determinants of health (SDoH) impacting our communities, pinpointing where inequities thrive.
Building upon this shared data, medical providers, CBOs, and patients collaborate to design strategic solutions and impactful interventions that address the identified needs. This may involve tailoring clinical care, creating culturally sensitive health programs, or advocating for policy change.
The hub fosters efficient coordinated action and communication. We’ll work together to streamline referrals, share best practices, and ensure seamless transitions between medical and social care services.
