SOMOS Community Care (SOMOS) is formed and submits it application to apply to the New York State DSRIP PPS program.
2015
SOMOS receives the 2nd highest score on its DSRIP PPS application enabling it to secure federal funding to implement chronic disease management programs.
2016
SOMOS deploys 100+ Community Health Workers and Physicians Engagement Specialists to support network practices and facilitate health education programs for the public.
2016
SOMOS implements multiple chronic disease management programs including the Asthma DASH Diet Plan, Educational and Tobacco Cessation program.
2017
SOMOS launches its first Behavioral Health program BUPE to address opioid abuse.
2018
SOMOS releases State of Latino Health report. The report is gives in-depth insight into the healthcare barriers Latino immigrants and Latino Americans face in New York City.
2019
SOMOS publishes the State of Chinese Health. The 1st study of its kind to examine the health disparities of Chinese immigrant and Chinese Americans in New York City.
2019
SOMOS becomes the 1st and only provider-led IPA to receive Innovator contract status in the DSRIP initiative.
2019
SOMOS launches the MiSOMOS health app.
2020
SOMOS successfully completed its five-year participation in the DSRIP program as the only provider-led PPS entity at the conclusion of the program.
2020
SOMOS opens and operates 51 multilingual COVID-19 testing sites. While providing testing at over 400 public schools.
2021
SOMOS operates more than 200 COVID-19 vaccination sites throughout NYC.
2021
SOMOS publishes SOMOS On the Frontline as a chronicle of its COVID-19 efforts.

SOMOS receives NY Emmy for the documentary “The Hispanic Physician Who Faced COVID-19”.

SOMOS launches initiative to destigmatize Mental Health and incorporate Behavioral Health into its wellness programs through health fairs and workshops.

SOMOS receives NY Emmy nomination for Basket In Anti-litter PSA.

SOMOS Community Care announces its partnership with the Pontifical Academy for Life for the Thank you, Doctor! Global Awareness campaign.
