Launching a Social Care Network
Starting in 2020 and concurrently through the pandemic response period, SOMOS built upon the PPS’s DSRIP accomplishments and furthered the value-based care initiatives, by developing a bonafide Social Care Network. In essence, SOMOS was investing in sustainable, critical infrastructure and programs such as Social Care despite the end of DSRIP and no clear extension of the waiver in sight.
During the past three years, SOMOS strengthened the network of physicians and partners, including a diverse network of Community-Based Organizations (CBOs)—leveraged effectively during DSRIP—that can reach, care for, and comprehensively address the social needs of the most vulnerable communities of New York City.
As the pandemic response ended, SOMOS initiated a full-fledged shift in programming efforts to the creation of a robust social care network that directly addresses health inequities disproportionately impacting the vulnerable communities it serves.
